Admission Registration Information
Our Registration Department would like to welcome you to Jackson Hospital and introduce you to our admission process, policies, and procedures. Whether you are planning an outpatient procedure or admission to our facility, we wish your visit to be a pleasurable and healthy experience while receiving the quality of care in which we take pride at Jackson Hospital.

When your physician schedules your appointment, certain information is required in order for us to begin your registration process. This information is discussed below. Please feel free to contact our registration department at any time you may have questions or require assistance at (850) 718-2663.

All information is kept confidential and will be verified upon your arrival to the hospital.

When You Arrive
Please arrive at least 15 minutes prior to your scheduled appointment to allow time for parking and reaching your destination. Our designated customer parking lots are located at the west entrance for pre-admissions, and one at the east entrance for outpatient services. Please check in at the information desk and you will directed to the appropriate registration area.

Insurance
It is important that you bring all insurance cards and proper identification with you in order to promptly complete the admission process. It is your responsibility to obtain any pre-admission or pre-testing authorizations that may be required by your insurance company.

So that you may be financially prepared: upon admission the hospital is required to collect any and all deductibles, coinsurance, and noncovered charges. The balance of these expenses should be paid prior to your discharge.

As a courtesy to you, we will file your insurance for you at the time of discharge. We allow 45 days for your insurance company to reimburse. However, if payment is not received within 45 days, you may receive a bill from the hospital expecting full payment at that time.

Financial Counselors are available from 8:00 a.m. to 4:30 p.m., Monday – Friday to assist you with any payment needs. Please call (850) 718-2641 and you will be directed to an appropriate financial counselor.

What to Bring
If you have an advance directive and/or living will, please bring a copy with you for your hospital records. We ask that you bring a minimum of personal belongings with you. Please leave any valuables at home. The hospital is not responsible for loss or damage to personal property. Please bring a current list of medications with you including the name of the medication, dosage, and prescribed instructions for taking your medication.

If you are being admitted to the hospital and will be staying overnight, it is suggested that you bring a robe, slippers, sleepwear, toilet articles, and any personal items for your comfort.

Instructions
If you are coming to the hospital for outpatient tests or surgery, be sure to follow any instructions given to you by your physician, i.e., no eating or drinking after midnight before the day of the procedure. If you are having a surgical procedure, you must arrange for a ride home. For your safety the hospital does not release a patient to leave alone, and you should not drive on the same day of a surgical procedure.
